1 July 2009 will mark the first anniversary of the Local Commercial FM Licence Agreements.
The Ministry requires all Local Commercial Licence Holders to complete and return a compliance report by 1 July 2009 in the form provided.
If an Amendment Agreement has not been entered into, please complete the Original Agreement Template.
If an Amendment Agreement has been entered into, please complete the Amended Agreement Template.
Please fill in and return by email or post by 1 July 2009.
Required documentation can be sent via post, word document or PDF file.
